The Home Health Licensed Practical Nurse (LPN) utilizes an individualized patient care plan to deliver top-quality care, aiming to keep patients Happy, Healthy, and at Home. The position is for the weekday team in Orange County, working Full-Time Monday-Friday from 8 am to 5 pm. LPNs will visit 6-7 patients per shift, traveling to their homes to provide skilled nursing services. The role is primarily responsible for the initial assessment and case development of patients across all life stages in their homes, including assessing, interpreting, planning, implementing, and evaluating patient care according to age and diagnosis. Strong assessment, skilled nursing, and documentation skills are encouraged. Clinicians document in patients' homes using company-issued laptops and must be comfortable traveling throughout the service area. Requirements

  • Licensed Practical Nurse (LPN)
  • Must be able to check your email intermittently during the week to respond to documentation requests.
  • LPNs must be okay with traveling throughout the service area!
  • Strong assessment, skilled nursing and documentation skills are strongly encouraged to apply!

Responsibilities

  • Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.
  • Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
  • Educate patients and families on health management, medications, and self-care techniques.
  • Collaborate with the healthcare team to ensure the highest quality care for each patient.
  • Visit 6-7 patients on any given shift, driving to the patients home providing skilled nursing services.
  • Primarily responsible for the initial assessment and case development of patients in all stages of life in their homes.
  • Assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis.
