The Home Health Licensed Practical Nurse (LPN) uses an individualized patient care plan to provide top quality care to all patients, keeping them Happy, Healthy and at Home. This position is primarily responsible for the initial assessment and case development of patients in all stages of life in their homes. Duties include but not limited to, assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis. LPNs must be okay with traveling throughout the service area and checking email intermittently during the week to respond to documentation requests. Clinicians document in the patients' homes with company-issued laptops.
Requirements
- Licensed Practical Nurse (LPN)
- Strong assessment skills
- Strong skilled nursing skills
- Strong documentation skills
- Must be able to check your email intermittently during the week to respond to documentation requests.
- LPNs must be okay with traveling throughout the service area!
Responsibilities
- Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.
- Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
- Educate patients and families on health management, medications, and self-care techniques.
- Collaborate with the healthcare team to ensure the highest quality care for each patient.
- Licensed Practical Nurse (LPN) visit 6-7 patients on any given shift, driving to the patients home providing skilled nursing services.
- Assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis.
