At Elara Caring, we’re seeking a Licensed Practical Nurse to deliver compassionate, skilled care in patients’ homes. You’ll build meaningful relationships through one-on-one visits while working closely with an RN Case Manager and a supportive clinical team.

Requirements

An active Massachusetts LPN license
Graduation from an accredited nursing program
At least one year of clinical nursing experience
A valid driver’s license, reliable transportation, and current auto insurance
Ability to travel between patient homes and assigned work locations
Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.

Responsibilities

Provide skilled nursing care according to each patient’s individualized care plan and physician orders.
Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
Teach patients and caregivers techniques that support health, safety, and independence at home.
Collaborate with the RN Case Manager as patient needs and care plans change.
Document visits, nursing interventions, and medication information accurately and promptly.
Communicate with physicians, patients, families, and the clinical team to keep care coordinated.
Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.
