About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, the Home Health Licensed Practical Nurse provides personalized, one-on-one care to help patients regain independence in their homes. The nurse works with physicians and other clinicians to manage care plans that support recovery. The role provides skilled nursing care in patients’ homes under the direction of an RN, implements individualized care plans, educates patients and families, documents care, and coordinates with the healthcare team.
Requirements
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
- Must undergo TB screening if selected for the patient-facing role.
- Must maintain personal vehicle liability insurance meeting applicable state minimum limits or the specified higher limits.
Nice To Haves
- Nursing experience in a Home Health or Hospice setting.
Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient visits, interventions, and outcomes.
- Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
- Participate in care coordination activities and discharge planning.
