About The Position

As a Home Health LPN, you’ll deliver skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician, and assigned by the supervising registered nurse. Whether you’re experienced in home health or transitioning from hospital, acute care or skilled nursing, this role offers the chance to practice at the top of your license with greater flexibility, autonomy and meaningful 1:1 patient relationship. Holy Cross Home Care and Hospice is a member of Trinity Health At Home, a national home care, hospice and palliative care organization serving communities across 11 states. As a faith-based, not‑for‑profit agency, we are committed to caring for patients and families in the place they feel most comfortable—at home. Our services include skilled nursing, physical, occupational and speech therapy and medical social work.

Requirements

Must have a minimum of one (1) year experience in an acute care setting.
Graduate from an accredited or state approved school of practical nursing
Current licensure in the State of Maryland
CPR certification required.
Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
Must have current Driver’s license and reliable transportation to and from work site.

Nice To Haves

Home healthcare experience preferred.
