Home Health - Licensed Practical Nurse (LPN)

We are seeking a licensed LPN who is compassionate, organized, and skilled in home care nursing to join our team. Requirements
- Current Colorado LPN or LVN License required.
- Valid Driver's License & clean Motor Vehicle Record (MVR)
Nice To Haves
- One year nursing experience in home health preferred
Responsibilities
- Plan, implement, and evaluate nursing care.
- Assist RNs with specialized procedures.
- Provide care per plan of care and agency policy.
- Monitor and report changes in client condition.
- Document each visit thoroughly.
- Teach clients appropriate self-care techniques.
