About The Position
SummitWest Care is a local nonprofit home health agency seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing services throughout Mesa County. Home health offers a different kind of nursing experience. Instead of balancing multiple call lights, constant interruptions, and large patient assignments, you'll provide one-on-one care where it matters most: in the patient's home. You'll enjoy the flexibility to help shape your schedule, the autonomy to work independently, and the support of an experienced clinical team invested in your success. Whether you're looking for part-time, PRN, or supplemental hours, this position offers the opportunity to build meaningful patient relationships while helping individuals remain safe, healthy, and independent at home.
Requirements
- Current Colorado Licensed Practical Nurse (LPN) License
- Valid driver's license, reliable vehicle, and auto insurance
- Strong communication and accurate documentation
- Ability to work independently and adapt professionally in home health environments
Nice To Haves
- IV Certification preferred; not required
Responsibilities
- Provide skilled nursing services in patients' homes
- Perform wound care, medication support, and patient monitoring
- Observe and report changes or concerns promptly
- Document visits accurately and timely in the EMR
- Collaborate with RNs, therapists, caregivers, families, and providers
- Support patient education and individualized care plan goals
