As we continue to grow, we're seeking a dedicated Licensed Practical Nurse (LPN) to join our Home Health team. This role is ideal for a nurse who is passionate about delivering one-on-one patient care, values autonomy in their work, and enjoys building meaningful relationships with patients and their families while making a lasting impact in the community. As a Home Health LPN, you'll provide skilled nursing visits in patients' homes. Working under the supervision of a Registered Nurse, you will: Deliver high-quality skilled nursing care according to physician orders and individualized care plans Monitor patient conditions and identify changes requiring intervention Provide wound care, medication management, disease education, and other skilled nursing services Educate patients and caregivers on self-care techniques, treatment plans, and safety measures Collaborate closely with RN Case Managers, physicians, therapists, and other members of the care team Document patient visits accurately and timely in the electronic medical record Assist in implementing care plans designed to improve outcomes and reduce avoidable hospitalizations Communicate significant findings and patient concerns to the interdisciplinary team Promote patient independence, safety, and quality of life in the home setting At Elara Caring, every team member plays a vital role in helping patients receive the care they need in the place they call home. Requirements
- Graduate of an accredited Practical Nursing program
- Current, unrestricted Licensed Practical Nurse (LPN) license
- Minimum of one (1) year of nursing experience in a clinical setting
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout the assigned service area
- Strong communication, assessment, and organizational skills
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

- Previous Home Health experience
- Experience working with adult and geriatric patient populations
- Experience with electronic medical record systems
