Home Health Licensed Practical Nurse (LPN) – (PRN)
About The Position
The Home Health Licensed Practical Nurse provides one-on-one, home-based care to patients with illnesses, injuries, and chronic diseases, often following hospital discharge. The LPN works under RN supervision and collaborates with physicians, RNs, CNAs, aides, and other members of the interdisciplinary team to support quality patient care and coordinate care and discharge planning. The role includes taking vital signs, conducting assessments, documenting patient progress, reporting changes to the RN supervisor, assisting with the patient’s plan of care, and teaching patients and families about self-care, medications, and home safety. This is a PRN position with per-visit rates of $35 for a regular visit and $40 for a hi-tech visit, with potential to transition to a full-time role in the future.
Requirements
- Graduate of an accredited practical nursing program.
- Active LPN license in Florida.
- At least 1 year of home healthcare experience.
- CPR or BLS certification.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing, strong communication skills, and compassion.
Responsibilities
