About The Position
Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN) in the East Hillsborough County, FL area. This role offers the opportunity to take control of your schedule while providing meaningful one-on-one care. The coverage area is Brandon / East Hillsborough County, FL, with territories designed to minimize drive time through clustered scheduling. The position offers a pay-per-visit model with average full-time earnings between $55,000 – $65,000+ annually, plus quarterly bonus opportunities and mileage reimbursement. Consistent patient volume is provided. The company emphasizes flexibility and autonomy, allowing nurses to create and manage their own daily schedules with options for full-time, PRN, or modified schedules. There are no hospital shifts or 12-hour rotations, promoting greater work-life balance with 1:1 patient care. The role utilizes technology such as AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) to streamline documentation, allowing nurses to finish their day sooner and spend more time with patients. Requirements
- Active, unencumbered Florida Licensed Practical Nurse (LPN) license
- Valid driver’s license, reliable transportation, and auto insurance
- CPR certification

Responsibilities
- Provide skilled nursing care under the supervision of a Registered Nurse
- Perform patient assessments and assist with care planning
- Administer medications and treatments as ordered
- Educate patients and caregivers on disease management and care plans
- Monitor patient condition and report changes to the supervising RN
- Collaborate with physicians and interdisciplinary teams
- Complete documentation using streamlined, AI-assisted tools
