Requirements

Graduate of an accredited practical nursing program and active LPN license in South Dakota
CPR certification
Knowledge of state and federal home health regulations
Compassionate, dependable, and committed to providing exceptional patient care.
Organized and able to manage their daily schedule independently.
A strong communicator who enjoys building relationships with patients and families.
A team player who values collaboration and continuous learning.
Comfortable using electronic medical records and completing timely documentation.
Reliable transportation, current driver's license, and current automobile insurance

Nice To Haves

Minimum of 1 year of LPN experience, ideally in home healthcare

Responsibilities

Providing skilled nursing visits in patients' homes under the direction of the RN Case Manager.
Performing nursing procedures and treatments ordered by the physician and outlined in the plan of care.
Monitoring patient conditions and reporting changes to the supervising RN.
Reinforcing education provided by the RN to help patients and families manage their health with confidence.
Administering medications and treatments within the LPN scope of practice.
Accurately documenting all patient care and clinical observations.
Collaborating with physicians, nurses, therapists, caregivers, and other members of the healthcare team.
Helping patients maintain independence, dignity, and the highest possible quality of life.
