Requirements
- Graduate of an accredited Practical Nursing program
- Current, unrestricted Licensed Practical Nurse (LPN) license
- Minimum of one (1) year of nursing experience in a clinical setting
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout the assigned service area
- Strong communication, assessment, and organizational skills
Nice To Haves
- Previous Home Health experience
- Experience working with adult and geriatric patient populations
- Experience with electronic medical record systems
Responsibilities
- Deliver high-quality skilled nursing care according to physician orders and individualized care plans
- Monitor patient conditions and identify changes requiring intervention
- Provide wound care, medication management, disease education, and other skilled nursing services
- Educate patients and caregivers on self-care techniques, treatment plans, and safety measures
- Collaborate closely with RN Case Managers, physicians, therapists, and other members of the care team
- Document patient visits accurately and timely in the electronic medical record
- Assist in implementing care plans designed to improve outcomes and reduce avoidable hospitalizations
- Communicate significant findings and patient concerns to the interdisciplinary team
- Promote patient independence, safety, and quality of life in the home setting
