Requirements

  • Graduate of an accredited Practical Nursing program
  • Current, unrestricted Licensed Practical Nurse (LPN) license
  • Minimum of one (1) year of nursing experience in a clinical setting
  • Valid driver's license, reliable transportation, and current auto insurance
  • Ability to travel throughout the assigned service area
  • Strong communication, assessment, and organizational skills

Nice To Haves

  • Previous Home Health experience
  • Experience working with adult and geriatric patient populations
  • Experience with electronic medical record systems

Responsibilities

  • Deliver high-quality skilled nursing care according to physician orders and individualized care plans
  • Monitor patient conditions and identify changes requiring intervention
  • Provide wound care, medication management, disease education, and other skilled nursing services
  • Educate patients and caregivers on self-care techniques, treatment plans, and safety measures
  • Collaborate closely with RN Case Managers, physicians, therapists, and other members of the care team
  • Document patient visits accurately and timely in the electronic medical record
  • Assist in implementing care plans designed to improve outcomes and reduce avoidable hospitalizations
  • Communicate significant findings and patient concerns to the interdisciplinary team
  • Promote patient independence, safety, and quality of life in the home setting
