About The Position
This role offers a unique nursing experience focused on one-on-one patient care, allowing for relationship development and schedule autonomy. As a Home Health LPN with Interim HealthCare of Oklahoma City, you will provide full attention to patients in their homes, a level of focus often difficult to find in other settings. This position is ideal for experienced home care nurses seeking greater independence or those new to home care looking for a fulfilling opportunity.
Requirements
- Active LPN license in Oklahoma
- CPR certification
- Reliable transportation and a valid driver's license
- Strong communication skills and a genuine passion for patient care
Nice To Haves
- Home health experience preferred, though we welcome LPNs looking to try something new
Responsibilities
- Provide skilled nursing visits to patients in their homes under the direction of the RN case manager
- Administer medications, treatments, and wound care per the plan of care
- Monitor patient condition and report changes to the RN and physician
- Educate patients and family members on care needs, medication, and safety
- Document visits accurately and timely to support continuity of care
- Collaborate with the care team, including RNs, aides, and therapy staff
