About The Position
The Home Health Liaison is responsible for coordinating patient care and discharge planning, primarily within an acute care setting and transitioning to home care. This role involves working with patients and families to ensure a smooth transition and continued care. The position is based at UW Health Charles St. Clinic.
Requirements
- Graduate of an accredited School of Nursing/College.
- At least one year professional nurse experience within the last five years in an acute care setting.
- Valid RN license.
- Valid State driver’s license.

- One year of experience in home care and hospital discharge planning.
Responsibilities
- Coordinate patient care and discharge planning from acute care to home care.
- Work with patients and families to ensure a smooth transition of care.
- Provide liaison services between the hospital and home health services.
