This is a Per Diem Home Health Intake Nurse position within the Patient Access department. The role is remote and requires availability Monday through Friday from 8 am to 6 pm, with weekend and holiday rotations. New hires will need to work 32-40 hours per week for the first 2-3 months during orientation. The position involves screening referrals to ensure patients meet agency admission criteria and comply with regulations, providing customer service, and coordinating patient transitions. The nurse will explain services, determine eligibility for Medicare-certified agencies, and refer to other community agencies when appropriate. Responsibilities also include identifying risk management situations, documenting the initial plan of care, and communicating with patients, families, physicians, and referral sources telephonically. The role requires knowledge of agency payors, Medicare, Medicaid, and contracted commercial insurance reimbursement guidelines, and providing education on services like DME, Hospice, Palliative Care, Pediatrics, and Home Infusion Therapy. The nurse may also participate in precepting new hires and suggesting improvements to current practices.
