The Home Health & Hospice Clinical Coordinator (LPN) provides skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload. This role involves reviewing caseloads with the supervising RN, obtaining physician orders, and collaborating with physicians as patient needs dictate. The coordinator evaluates assessment findings, communicates pertinent patient information to the healthcare team, and educates patients, caregivers, families, and staff. They ensure patient safety, participate in case conferences, recommend referrals, and follow medication rights to minimize errors. Additionally, they follow up on lab results, participate in the agency's Quality Assurance Performance Improvement (QAPI) program, identify and mitigate environmental risks, and complete accurate visit notes via Electronic Health Record (EHR).
