About The Position
Franciscan Healthcare is seeking a compassionate and dedicated Home Health and Hospice Aide to join our Home Health and Hospice team where every interaction is an opportunity to make a meaningful difference. In this vital role, you’ll provide hands-on support to patients in the comfort of their own homes, working under the guidance of licensed medical professionals. You’ll play a key part in enhancing quality of life by assisting with personal care, promoting comfort and safety, supporting daily activities, and performing essential nursing tasks. Requirements
- Nursing Assistant or Hospice Aide certification through an approved program
- Valid driver’s license
- Interpersonal skills requiring courtesy and tact are necessary to communicate effectively
- Medium strength with lifting of 50 lbs. occasionally, 25lbs. frequently, and 10 lbs. constantly (Lifting over 35 lbs requires assistive devices be used)
- Speak and understand the English language
- Ability to properly lift position and transport patients (with assistance as defined)
- Ability to stabilize unsteady patient when ambulating
- Ability to record information accurately and follow instructions exactly
Nice To Haves
- Experience in hospice or related health care field preferred
Responsibilities
- Ensure quality and safe delivery of personal care services
- Participate in the development and implementation of the plan of care
- Implement current personal care services; discuss plan of care with RN
- Provide patient care in regard to hygiene, mobility, comfort, psychosocial, spiritual and nutritional needs
- Carry out treatments, collection of specimens, vital signs, intake and output, and monitoring and emptying drainage devices
