About The Position

The Home Health Field Registered Nurse (RN) plays a vital role in delivering exceptional, patient-centered care in the comfort of patients’ homes. This position is responsible for comprehensive assessment, care planning, implementation, and evaluation of individualized nursing care while managing complex medical needs. As part of an interdisciplinary team, the RN collaborates with physicians, therapists, caregivers, and families to promote recovery, manage chronic conditions, prevent complications, and help patients maintain independence and quality of life.

Requirements

  • Current Registered Nurse (RN) License
  • Valid Driver’s License
  • Current CPR Certification
  • Valid Auto Insurance

Nice To Haves

  • Minimum of 2 years of Home Health RN experience preferred
  • HomeCare HomeBase experience preferred

Responsibilities

  • Perform initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs through clinical documentation, evaluations, care conferences, and ADL assessments.
  • Apply admission and recertification criteria to determine eligibility and appropriate levels of home health care services.
  • Develop, implement, and update individualized plans of care to ensure quality outcomes and continuity of care.
  • Educate patients and caregivers on disease management, medications, self-care techniques, nutrition, and lifestyle modifications.
  • Determine visit frequency and service scope based on patient acuity and caregiver support.
  • Evaluate caregiver capability and provide support throughout the continuum of care.
  • Initiate preventive, restorative, and rehabilitative nursing interventions.
  • Utilize infection prevention practices and standard precautions during patient care activities.
  • Supervise and provide clinical direction to Home Health Aides and LVN/LPN team members.
  • Complete timely and accurate clinical documentation and progress notes.
  • Communicate changes in patient condition to physicians and care team members while evaluating patient response to interventions.
  • Monitor assigned cases for compliance with payer requirements and agency standards.
  • Maintain effective communication with patients, families, interdisciplinary teams, and referral partners.
