The Home Health Field Registered Nurse (RN) delivers high-quality, patient-centered care in patients’ homes. The role includes comprehensive assessment, planning, implementation, and evaluation of nursing care tailored to individual needs, including care for complex medical conditions. The RN collaborates with interdisciplinary teams, patients, and families to promote health, prevent complications, and support recovery or chronic disease management. Through education, emotional support, and clinical expertise, the RN helps patients achieve optimal health outcomes and maintain independence, supporting continuity of care outside traditional clinical settings. This is a pay-per-visit role. The RN assesses patient needs, determines appropriate service scope and frequency, develops and documents care plans, provides and coordinates nursing care, and communicates changes and outcomes to physicians and other care team members. The RN also provides clinical direction to Home Health Aides and LPNs/LVNs and monitors assigned cases for third-party payor compliance.
