Requirements
- Current RN license in the state of practice
- Valid driver's license and auto insurance
- Current CPR certification

- Home Health experience preferred
- HomeCare HomeBase experience preferred
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Provide skilled nursing care and ongoing patient evaluations.
- Educate patients and caregivers on disease management, medications, nutrition, and self-care.
- Collaborate with physicians and interdisciplinary team members to coordinate care.
- Monitor patient progress and adjust care plans as needed.
- Document clinical findings and maintain compliance with home health regulations.
- Supervise and support Home Health Aides and LVN/LPN team members.
- Promote patient safety, infection prevention, and successful recovery at home.
