Requirements

Current Registered Nurse (RN) license in the applicable state
Valid driver's license and reliable transportation
Current auto insurance
Current CPR certification

Nice To Haves

Minimum of two (2) years of Home Health RN experience preferred
HomeCare HomeBase experience preferred

Responsibilities

Perform comprehensive patient assessments, including physical, functional, psychosocial, and environmental evaluations.
Develop, implement, and update individualized plans of care based on patient needs and physician orders.
Determine patient eligibility for admission and recertification while ensuring compliance with home health regulations.
Provide skilled nursing care, treatments, and patient education focused on disease management, medications, nutrition, and self-care.
Collaborate with physicians, therapists, caregivers, and the interdisciplinary team to ensure seamless continuity of care.
Supervise and provide clinical direction to Home Health Aides and LVNs/LPNs.
Monitor patient progress, communicate changes in condition, and adjust care plans as appropriate.
Maintain accurate, timely clinical documentation and progress notes.
Follow infection prevention and safety standards while delivering high-quality patient care.
Ensure compliance with payer, regulatory, and agency requirements.
