Full - Time Home Health Field RN Case Manager
About The Position
Amcare Pro Home Health is a mid-size home health care company with offices in DFW and East Texas. We are currently interviewing for a Full Time Home Health Field RN Case Manager to cover the Collin County (Plano, Allen, Frisco, McKinney & surrounding areas). The position will involve home care visits for Admits, Recerts, Sup & SN visits. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/HOME CARE HOME BASE/AXXESS) is a plus. The RN position will be required to perform home health SN Initial Assessments, OASIS Admissions, Re-cert & ROC visits, DC visits ensure compliance with all State regulations. The RN must have knowledge/experience with home health forms, OASIS charting, Medicare guidelines, rules & regulations.
Requirements
- Must currently be licensed in the state of Texas
- Should have at least 2 year nursing experience
- Prior home health care experience is a MUST
- Should have Reliable Transportation & Car insurance
- Must be able to pass background check & have valid CPR
- English (Required)
- Geriatrics: 2 years (Required)
- Home health: 2 years (Required)
- RN License (Required)
- Driver License
Nice To Haves
- Familiarity with electronic charting (KINNSER/HOME CARE HOME BASE/AXXESS) is a plus.
- IV infusion experience is a plus
- Experience with Oasis E and use of Wellsky/Kinnser is a plus
- Knowledge of ICD-10 codes is a plus
- Spanish (Preferred)
- Certified Registered Nurse Infusion (Preferred)
- Wound Vac Certification (Preferred)
- CPR/BLS Certification (Preferred)
Responsibilities
- Conduct SOC, ROC, recertification, and discharge visits
- Complete OASIS assessments accurately and timely
- Develop and manage individualized Plans of Care
- Provide skilled nursing care (wound care, medication management, disease management, IV therapy as needed)
- Coordinate care with physicians and interdisciplinary team
- Educate patients and caregivers on treatment plans and safety
- Ensure compliance with Medicare and Texas home health regulations
- Complete all documentation in the agency EMR within required timeframes
