About The Position

21st Century Home Health Services is hiring a field Registered Nurse to provide skilled nursing care to patients in their homes in the Sacramento area. The role begins with follow-up visits under the guidance of an experienced Case Manager RN, with support while learning the organization’s processes. As the nurse gains confidence, the role can grow to include more comprehensive visits and hands-on exposure to case management. All opportunities require field work and home visits. The position is full-time, five days per week, with schedule options of Tuesday–Saturday, Sunday–Thursday, or Wednesday–Sunday. Weekend coverage is focused primarily on admissions and starts of care. Nurses are expected to average 35 or more visits per week and participate in a shared weekend on-call rotation. The nurse is assigned a primary territory, but temporary coverage outside that territory may be requested to support staffing gaps, leaves, increased census, or other patient-care needs. The nurse works with a dedicated Case Manager RN, participates in weekly case conferences, and uses AI-powered charting tools to help complete documentation. Compensation follows a pay-per-visit model; the posted estimated wage range is $130,000–$175,000 per year, with starting pay based on relevant experience above the minimum requirements. The company offers comprehensive health benefits and a 401(k) plan with up to a 4% match.
