About The Position

Deliver meaningful care where it matters most—at home. The Home Health Field RN provides skilled, patient-centered nursing care in the home setting, supporting patients with acute and chronic conditions while promoting independence, safety, and quality of life. This role partners closely with physicians, families, and interdisciplinary teams to ensure continuity of care and optimal outcomes outside the hospital environment.

Requirements

• Active Registered Nurse (RN) license
• Valid driver’s license, reliable transportation, and current auto insurance
• Current CPR certification

Nice To Haves

• 2+ years Home Health RN experience preferred
• HomeCare HomeBase experience preferred
• Strong clinical judgment, autonomy, and time management skills

Responsibilities

• Complete comprehensive initial and ongoing patient assessments (physical, functional, psychosocial, environmental)
• Establish eligibility and determine appropriate level of home health services per admission and recertification criteria
• Develop, implement, evaluate, and revise individualized Plans of Care
• Provide skilled nursing interventions, preventative care, and chronic disease management
• Educate patients and caregivers on medications, self-care, nutrition, and disease process
• Coordinate care with physicians and interdisciplinary team members
• Supervise and direct Home Health Aides and LPN/LVNs as appropriate
• Document clinical notes accurately and timely, reflecting patient progress and outcomes
• Monitor compliance with payer requirements and care guidelines
• Communicate changes in condition promptly to physicians and care team
