About The Position

This role is for a compassionate, skilled Home Health LVN/LPN who thrives in a field-based environment and is passionate about delivering personalized, high-quality care. As a Home Health LVN/LPN, you’ll play a vital role in helping patients recover, maintain independence, and manage chronic conditions—all from the comfort of their homes.

Requirements

• Current LVN/LPN license (active and in good standing)
• Valid driver’s license & auto insurance (field role)
• CPR certification

Nice To Haves

• Home health experience preferred
• Experience with HomeCare HomeBase (HCHB) is a plus

Responsibilities

• Provide direct patient care under the supervision of an RN
• Assist in assessments of physical, psychosocial, and environmental needs
• Implement and support individualized care plans (POC)
• Perform medication administration, wound care, and treatments
• Educate patients and families on disease management and care routines
• Ensure continuity of care through clear, timely documentation
• Collaborate with physicians, therapists, and interdisciplinary team members
• Follow infection control and safety protocols at all times
• Maintain compliance with regulatory and payer requirements
