About The Position

Choice Health at Home is hiring immediately for a Home Health Field LVN/LPN to provide compassionate, high-quality care to patients in the comfort of their homes. In this role, you’ll work closely with Registered Nurses, physicians, therapists, patients, and families to support individualized plans of care. You’ll help patients safely manage their health conditions, promote independence, and achieve better outcomes while recovering or managing chronic conditions at home.

Requirements

Current and valid LVN/LPN license
Valid Driver’s License
Valid Auto Insurance
Current CPR Certification

Nice To Haves

Home Health experience preferred
Homecare Homebase (HCHB) experience preferred

Responsibilities

Assist the RN in identifying patients’ physical, psychosocial, and environmental needs through assessments, clinical documentation, team reports, and evaluations.
Participate in planning and implementing patient care in conjunction with the RN and in accordance with the established Plan of Care (POC).
Provide skilled nursing care and monitor patients for changes in condition.
Apply infection control practices and standard precautions while providing patient care.
Help ensure continuity of care through the delivery of safe, high-quality patient services.
Communicate effectively with patients, families, physicians, team members, and other healthcare professionals.
Maintain timely and accurate clinical documentation, communication notes, and required records.
Monitor assigned cases to help ensure compliance with third-party payer requirements.
Educate patients and caregivers regarding care needs, treatment plans, safety, and health maintenance.
Promote Choice Health at Home’s philosophy, policies, and commitment to quality patient care.
