About The Position

As a Home Health LVN/LPN, you’ll practice at the top of your license—delivering skilled, one-on-one care in patients’ homes. You’ll work independently in the field, while staying connected to a supportive and collaborative care team.

Requirements

• Current LVN/LPN license (required)
• Valid driver’s license and auto insurance
• Current CPR certification

Nice To Haves

• Home health experience preferred
• Experience with HomeCare HomeBase (preferred)

Responsibilities

• Partner with the RN to develop and implement individualized patient care plans
• Deliver skilled nursing care, including medication administration and wound care
• Educate patients and caregivers on treatment plans and disease management
• Monitor assigned caseload to ensure continuity, quality, and compliance with the Plan of Care
• Track patient progress, identify changes in condition, and communicate updates to the RN and physician
• Collaborate with the interdisciplinary team to ensure high-quality, coordinated care
