Requirements

• Current LVN/LPN license in the applicable state
• Valid driver's license and reliable transportation
• Current auto insurance
• Current CPR certification

Nice To Haves

• Home health experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Deliver skilled nursing care under the supervision of a Registered Nurse in accordance with the physician's plan of care.
• Assess and report patients' physical, psychosocial, and environmental needs.
• Assist in developing, implementing, and updating individualized care plans.
• Administer medications and treatments while following infection prevention and safety protocols.
• Educate patients and caregivers on disease management, medications, and home care techniques.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, therapists, caregivers, and the interdisciplinary team to ensure continuity of care.
• Monitor assigned patient cases to ensure compliance with payer and regulatory requirements.
• Promote the highest standards of patient care while supporting the mission and values of Choice Home Health.
