This position requires the ability to assess patient health status, administer medications, and provide wound care while collaborating closely with physicians, therapists, and family members. The LVN/LPN will be responsible for developing and implementing individualized care plans that promote patient independence and improve overall well-being. This role demands strong clinical judgment and compassionate communication to educate patients and caregivers on treatment protocols and health maintenance. Ultimately, the Home Health Field LVN/LPN contributes to enhancing patient outcomes by providing personalized, attentive care in a home setting, supporting recovery and chronic disease management outside of traditional clinical environments.

Requirements

• Current LVN License
• Valid Driver’s License
• Valid Auto Insurance
• CPR Certification

Responsibilities

• Provide skilled, hands-on nursing care to residents right where they live
• Work closely with your RN care coordinator to carry out care plans and share updates on patient progress
• Build strong, friendly relationships with residents, families, and community staff
• Join in at community wellness events and health fairs when you'd like to (cake is often involved)
• Keep accurate, timely documentation so care stays seamless
• Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
• Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
• Assure the continuity of care through delivery of quality patient care.
• Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with requirements of third party payor.
• Promote Agency philosophy and administrative policies to ensure quality of care.
