About The Position

Provides direct occupational therapy to patients in their residence. Implements plan of care. Adheres to the Code of Conduct and upholds the philosophy, vision, and core values of the organization and ensures the Christian Mission of EverTrue is realized so that “Older Adults Live Life to the Fullest”.

Requirements

Valid state licensure to work as an Occupational Therapy Assistant
Current CPR certification
Ability to work independently
Knowledge of Microsoft Office or equivalent programs
Excellent customer service skills
Excellent problem resolution skills
Detail oriented and good organizational skills

Responsibilities

Assists OTR in developing and implementing individual treatment goals which reflects patients’ needs
Assists OTR in developing an OT treatment plan on each patient. Reviews and consults with OTR frequently for needed changes
Following review with OTR, communicates with other members of medical care team and personnel of other departments in providing for patient’s total needs; This includes recommending other ancillary service referrals
Implements treatment plan as outlined by OTR and reports progress in medical record on each visit
Provide support and education for patient and family members regarding patient’s OT needs
OTR communicates with physician, home care nurse, OTR, promptly on any changes in patient’s physical or emotional condition, or any significant occurrences
Attends and participates in case conferences and agency in-service programs
Provide input in establishing department polices
Promote awareness and utilization of home care services to other professionals, physicians, and the community
Participates as a member of the home care team
