About The Position

This is a part-time Home Health CNA position covering Racine/Kenosha county. The role requires flexibility with days Monday-Friday from 8:00 AM to 4:30 PM, with no holidays or weekends. The pay range is $20.80 - $31.20 per hour. The position contributes to effective patient outcomes by providing competent patient care assigned, including assisting with ADLs, noting changes in patient condition, understanding hospice philosophy, performing personal care needs, meeting safety needs, taking vital signs, providing skin care, and changing linens. The role also involves conducting general work activities consistently, demonstrating the ability to work independently, attending to personal work schedule professionally, cooperating with team members, and being secure with issues of death and dying while providing a calm manner. Effective communication skills are essential, including utilizing appropriate communication and interpersonal skills, participating in achieving patient outcomes, preparing documentation according to agency standards, and maintaining patient confidentiality. Active participation in the CQI Process, adherence to regulatory requirements, and demonstrating professionalism and cooperation with all associates are also key aspects of this role.

Requirements

  • High school education or equivalent.
  • One year nurse’s aide experience.
  • RN nursing students that meet state requirements.
  • Recognize the needs and behaviors of a variety of age groups of patients treated, understand child growth and development, i.e. understand adolescent behavior and look for signs of normal aging.
  • Written and verbal competency of the English language.
  • Must be certified as a nursing assistant in the state in which the teammate works
  • A valid driver’s license issued by the Division of Motor Vehicles.

Responsibilities

  • Demonstrates competent home health aide skills.
  • Promotes patients to reach functional status goals by providing assistance with ADL’s following the HHA assignment record in rendering patient care and maintaining safety.
  • Notes changes in patient’s condition requiring further attention, informs the case manager, or the RN/Supervisor and documents accordingly.
  • Understands the hospice philosophy, comfortable providing specialized care to the terminally ill.
  • Performs personal care needs, feeding patient, laundry, light cleaning of patient room and washing.
  • Meeting safety needs of patient using equipment safely and properly.
  • Taking temperatures, pulse, respiration and blood pressure as directed by the home health aide assignment sheet when assigned.
  • Provides skin care and changing patient position to prevent breakdown as assigned.
  • Changing patient bed linens and clothing as requested by the patient and family.
  • Exhibits the ability to work independently.
  • Attends to personal work schedule in a consistent and professional manner.
  • Demonstrates ability to cooperate and work well with team members.
  • Secure with issues of death and dying. Provides a calm manner when in a patient’s home.
  • Utilizes appropriate communication skills and effective interpersonal skills with all customers.
  • Actively participates in achieving patient outcomes through case conferencing and IDG’s.
  • Prepares documentation reflecting patient’s care; adheres to documentation as required by agency, state, federal, and Accrediting Organization standards with time frames.
  • Maintains patient confidentiality, observing of HIPPA/ Privacy acts.
  • Demonstrates cost-effective measures in patient care.
  • Assists with problem identification and solutions.
  • Participates in quality, improvement projects when assigned.
  • Demonstrates measures to ensure patient satisfaction.
  • Maintains annual inservice hours, Hospice education hours and OSHA requirements.
  • Cooperates in scheduling of required supervisory visits.
  • Provides home health aide services in accordance with all regulatory rules and standards.
  • Meets the training requirements of State and HIPPA.
  • Demonstrates flexibility in scheduling to support department and other service offices when needed.
  • Identifies own developmental needs and contributes to learning plan.
  • Demonstrates self-direction for maintaining home health aide skills.
  • Demonstrates commitment and supports team, office, company and organizational goals.
