This role will support Mequon, Brown Deer and Glendale Areas Monday-Friday 8:00am-Friday. No weekends. The Home Health Aide is responsible to provide competent patient care assigned. They contribute to effective patient outcomes by providing assistance with ADL’s, meeting safety needs of patients, taking vital signs, providing skin care, and changing linens. The role requires the ability to work independently, attend to personal work schedule professionally, cooperate with team members, and be comfortable with issues of death and dying. Effective communication skills are essential, including utilizing appropriate communication and interpersonal skills, participating in case conferencing, and preparing accurate documentation while maintaining patient confidentiality. The position also involves active participation in the CQI process, demonstrating cost-effective measures, assisting with problem identification and solutions, and participating in quality improvement projects. Adherence to regulatory requirements, including annual inservice hours, Hospice education hours, OSHA requirements, and state/HIPAA training, is mandatory. Professionalism and cooperation with all associates are expected, including flexibility in scheduling, identifying developmental needs, demonstrating self-direction, and supporting organizational goals.
