About The Position
Inspired Homecare is looking for a Home Health Clinical Manager to oversee all patient care and clinical personnel, ensuring care is coordinated, compliant, timely, and aligned with individualized plans of care and agency standards. The Clinical Manager plays a vital role in upholding these standards and ensuring our patients receive the highest quality of care.
Requirements
- Registered Nurse (RN) with current state license
- 2-3 years of home health experience required
- Strong leadership, communication, and problem-solving skills
- CPR Certification, valid driver's license and auto insurance
Responsibilities
- Supervise all patient care services and staff (RN, LPN, OT, PT, HHA)
- Coordinate patient care and referrals
- Ensure ongoing assessment of patient needs
- Provide direct patient care when staffing requires
- Support clinicians with OASIS, documentation and case management
- Review clinical documentation
- Lead case conference, clinical meetings, and in-services
- Ensure patient charts and records are accurate, and regulatory compliant
- Monitor clinical outcomes
