About The Position

Facilitates and coordinates client care provided by all disciplines, ensuring the delivery of quality care in compliance with policies, procedures, CMS Conditions of Participation (CoP’s), and Department of Health regulatory requirements. Provides supervision to all staff and assumes total responsibility of the agency in the Director's absence. This role involves administrative and financial responsibility, planning, coordinating, managing, and evaluating the activities of a multidisciplinary team to ensure high-quality home care services. The position requires the ability to analyze large amounts of information, develop protocols, and ensure adherence to all agency policies and state/federal regulations.

Requirements

  • Graduate of an accredited school of nursing.
  • BSN preferred, AAS with commensurate clinical experience accepted.
  • PA or NY licensure.
  • A minimum of three years clinical home health experience.
  • Demonstrated abilities in Medicare Certified Home Health Agency (CHHA) standards.
  • Demonstrates competency in leadership skills and clinical nursing practice.
  • Minimum of one year CHHA management experience required.

Responsibilities

  • Facilitate and coordinate client care provided by all disciplines.
  • Provide supervision to all staff, ensuring the delivery of quality care.
  • Ensure compliance with policies and procedures, CMS Conditions of Participation (CoP’s), and Department of Health regulatory requirements.
  • Participate in the assessment, maintenance, and development of staff competencies.
  • Collaborate with Quality Manager/Clinical Educator to identify educational criteria.
  • Encourage development of leadership skills in staff.
  • Responsible for recruiting, interviewing, and hiring new employees.
  • Provide clinical expertise for administrative decision-making.
  • Provide support to staff related to the use of MatrixCare.
  • Monitor all orientation programs weekly.
  • Work closely with Director and be accountable for administrative and financial responsibility of the agency.
  • Assume total responsibility of the agency for the Director in their absence.
  • Plan, coordinate, manage and evaluate the activities of a multidisciplinary team.
  • Develop protocols and procedures to improve staff productivity.
  • Assure the implementation and administration of all agency policies.
  • Ensure all staff final payroll is processed in a timely manner.
  • Conduct case conferences and contribute education in Staff Meetings.
  • Assist Director in preparation for Staff Meetings, PAC Meetings and Board Meetings.
  • Assure that the agency is practicing according to State Department of Health and CMS Federal regulations, Conditions of Participation (CoP’s).
  • Assist Quality Manager/Clinical Educator in assessment and provision of competencies for all staff.
  • Serve on committees as assigned.
  • Travel for this position may be required.
  • Encouraged to participate in community activities.
