Interim HealthCare is seeking a Clinical Manager to oversee and elevate the patient care services at our agency. In this pivotal role, you’ll be responsible for leading patient and personnel assignments, coordinating care and referrals, ensuring continuous assessment of patient needs, and guiding the development of individualized care plans. Requirements

  • Current RN license in South Carolina or compact multi-state license.
  • RN license in good standing.
  • Graduate of an accredited nursing program.
  • Strong background in clinical practice and home care management.

Nice To Haves

  • 3+ years of home care experience.
  • 2+ years of management experience.

Responsibilities

  • Lead and support an interdisciplinary clinical team, ensuring the delivery of high-quality, patient-centered home health care.
  • Oversee patient assessments, individualized plans of care, and coordination of physician-ordered services.
  • Supervise, mentor, and evaluate clinical staff while promoting regulatory compliance and best nursing practices.
  • Monitor clinical documentation, patient outcomes, and quality metrics to ensure accuracy, timeliness, and agency standards are met.
  • Collaborate with physicians and the care team to coordinate referrals, transitions of care, and discharge planning.
  • Educate and support patients and caregivers to promote understanding of care plans, disease management, and health goals.
  • Lead Quality Assessment and Performance Improvement (QAPI) initiatives and conduct supervisory home visits to ensure exceptional care delivery.
