About The Position

All About Home Care is a Houston-based home health care agency that has been serving Houston and the surrounding communities since 2004, providing quality home health services. We are currently looking for PRN Certified Occupational Therapy Assistants (COTA) to help care for patients across the Greater Houston area, with coverage available in multiple areas including North, South, East, West, and Central Houston. We use Kinnser software to document patient care.

Requirements

Graduate of an accredited Occupational Therapy Assistant program
Current Texas state license as a COTA

Nice To Haves

Two years of experience in Home Health preferred
Spanish-speaking preferred

Responsibilities

Adhere to established agency policies and procedures
Provide physician-prescribed occupational therapy services under a plan of care established by an Occupational Therapist (OT)
Help patients improve or maintain their ability to perform activities of daily living (ADLs)
Support patients in regaining independence and achieving their highest functional potential
Carry out skilled therapy procedures as ordered by the physician and directed by the OT
Collaborate with the OT and other healthcare personnel in patient care planning
Report changes in patient condition and progress to the supervising OT
Educate patients and family members/caregivers on home programs and ADL strategies
Participate in mandatory in-service and performance improvement activities as assigned
Attend patient care conferences as scheduled
Document timely and accurate visit notes, progress updates, and care plan revisions as per agency requirements
