Job Details Join the Cleveland Clinic team, where you will work alongside passionate caregivers and provide patient-first healthcare. A caregiver in this role will serve patients in the following cities: Cleveland, Parma, Brook Park, Old Brooklyn, Lakewood, Parma Heights and others as needed Cleveland Clinic’s Home Care team helps patients continue their treatment and learn/relearn any skills necessary to stay safe and independent again. As an RN, you are responsible for providing direct nursing care, participating in a team-based care model, implementing and monitoring patient care plans, and reporting patients' needs after every visit. A caregiver in this position works from 8:00AM - 5:00PM, with on-call requirements.

Requirements

Graduate from an accredited school of Professional Nursing
Current state licensure as a Registered Nurse (RN)
One year of current clinical experience as an RN
One year of experience in an acute medical/surgical or homecare setting
Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage

Responsibilities

Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
Participate in a team-based care model led by the Care Coordinator in each region.
Implement and monitor patient care plans established by, and report patient needs after each visit to, the Care Coordinator for further case management.
Perform accurate and complete starts of care, resumptions, recertification's and routine visits as requested and within specified time frames.
Perform medication reconciliation at each visit.
Assess patients for safety risks, abuse and neglect.
Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
Report abnormal findings and/or changes in patient condition to the Physician and update Care Coordinators with those changes and any follow-up needed.
Communicate any patient's fall out/missed visits to the Care Coordinator immediately.
Participate in and support quality improvement projects and participate in department committees as assigned.
