Job Details Join the Cleveland Clinic team, where you will work alongside passionate caregivers and provide patient-first healthcare. A caregiver in this role will serve patients in the following cities: Cleveland, Parma, Brook Park, Old Brooklyn, Lakewood, Parma Heights and others as needed Cleveland Clinic’s Home Care team helps patients continue their treatment and learn/relearn any skills necessary to stay safe and independent again. As an RN, you are responsible for providing direct nursing care, participating in a team-based care model, implementing and monitoring patient care plans, and reporting patients' needs after every visit. A caregiver in this position works from 8:00AM - 5:00PM, with on-call requirements.

Requirements

  • Graduate from an accredited school of Professional Nursing
  • Current state licensure as a Registered Nurse (RN)
  • One year of current clinical experience as an RN
  • One year of experience in an acute medical/surgical or homecare setting
  • Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage

Responsibilities

  • Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
  • Participate in a team-based care model led by the Care Coordinator in each region.
  • Implement and monitor patient care plans established by, and report patient needs after each visit to, the Care Coordinator for further case management.
  • Perform accurate and complete starts of care, resumptions, recertification's and routine visits as requested and within specified time frames.
  • Perform medication reconciliation at each visit.
  • Assess patients for safety risks, abuse and neglect.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Report abnormal findings and/or changes in patient condition to the Physician and update Care Coordinators with those changes and any follow-up needed.
  • Communicate any patient's fall out/missed visits to the Care Coordinator immediately.
  • Participate in and support quality improvement projects and participate in department committees as assigned.
