About The Position

Pasco Southwest Home Health is seeking a dedicated Home Health Caregiver to provide essential personal care services to clients in their homes. This part-time, on-site role focuses on helping clients maintain their independence and dignity. The agency is committed to providing life-changing home health services in the Cortez, Grand Junction, and Southern Colorado areas and is known for its supportive and compassionate work environment.

Requirements

  • High school degree required.
  • Completion of appropriate training program.
  • Successful completion of skills competency evaluation.
  • Ability to read and follow written instructions and document care given.
  • Self-directing with the ability to work with little direct supervision.
  • Empathy for the needs of the ill, injured, frail and the impaired.
  • Demonstrates tact, patience, and good personal hygiene.
  • Keep the IHSS RN/IHSS Team Lead informed of progress considerations and any pertinent information regarding the client’s care.
  • Valid, current Colorado driver’s license and access to a reliable automobile or access to public or other reliable transportation.
  • Licensed driver with automobile that is insured in accordance with Organization requirements and is in good working order.

Nice To Haves

  • Six months or more of aide experience in a home care setting is preferred.
  • Knowledge of principles and procedures of personal care and safety practices in the home care setting.
  • Understanding of family interactions and aging process.
  • Beginning knowledge of nutrition and food service.
  • Ability to communicate effectively, verbally and in writing.
  • Ability to establish and maintain effective working relationships with clients and families, staff members and the general public.

Responsibilities

  • Provide personal care including baths, oral hygiene, back rubs, and assist with dressing and undressing clients.
  • Assist with toileting activities and skin care to prevent breakdown.
  • Maintain cleanliness and orderliness of the client’s living area.
  • Plan and prepare nutritious meals and assist with feeding when necessary.
  • Observe clients closely and document care provided and any changes in condition.
  • Travel to assigned client’s individual homes to provide care.
  • Complete cell Trak time keeping accurately and punctually.
  • Complete incident reports as deemed necessary.
  • Submit progress notes, changes in condition and report important changes observed in the client s home.
