Requirements
- Current RN license in the state of Massachusetts
- Strong technology skills
- Reliable transportation, valid driver's license, auto insurance
- Strong clinical assessment and documentation skills
Responsibilities
- Delivers skilled nursing care to patients in their home in accordance with individualized care plans.
- Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services.
- Coordinates services, informing the physician, management, and other personnel of changes in the patient's condition and needs.
- Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards.
- Conducts ongoing medical documentation, writing OASIS assessments as appropriate.
- Prepares clinical and progress notes, submitting all required paperwork daily.
- Responds promptly to emails and phone calls.
