About The Position
This is a true outside sales role, not a desk job. It's a boots-on-the-ground healthcare sales position where you will be walking into doctor’s offices, hospitals, SNFs, and assisted living facilities, building relationships face-to-face, and asking for referrals and earning business daily. Guardian Angel Home Care is a Medicare-certified home health agency operating across multiple states. We partner with healthcare providers to ensure patients receive high-quality care in the comfort of their homes. Your role is to drive that connection.
Requirements
- Experience in healthcare sales, home health, hospice, pharma, SNF/ALF outreach, or similar
- Strong communication and relationship-building skills
- Ability to work independently and manage a territory
- Valid driver’s license and reliable transportation
- CRM experience is a plus
Responsibilities
- Generate patient referrals from: Hospitals, Skilled Nursing Facilities (SNFs), Physician offices, Assisted living communities
- Coordinate patient transitions into home health services
- Build and maintain strong referral relationships
- Obtain physician orders and required documentation
- Communicate with clinical teams to ensure smooth admissions
- Execute a structured daily and weekly territory plan
- Track all activity and referrals in CRM (Salesforce or similar)
- Represent the company professionally at all times
