About The Position

Choices Home Health is seeking a motivated Sales Liaison who thrives in the field, knows the local market, and is ready to own their territory and drive real growth. This role involves developing and maintaining high-value referral relationships, positioning Choices Home Health as a leading provider through strategic outreach, identifying new accounts, and collaborating with clinical teams for seamless patient transitions. The ideal candidate will stay informed about market trends, execute marketing strategies, and consistently meet or exceed sales goals.

Requirements

  • Minimum of 2 years home health or hospice sales experience preferred
  • CRM Software experience preferred
  • Valid Driver’s License
  • Valid Auto Insurance
  • HomeCare HomeBase experience preferred

Responsibilities

  • Develop and maintain high-value referral relationships that consistently generate business
  • Position Choices Home Health as the go-to provider through strategic, in-person outreach
  • Identify new accounts, open doors, and close referral opportunities
  • Collaborate with clinical teams to ensure smooth patient transitions and exceptional service delivery
  • Stay ahead of market trends and competitor activity
  • Execute targeted outreach and marketing strategies that drive admissions
  • Consistently meet or exceed sales/admission goals
