Requirements
- Active RN license (CT)
- 1+ year of nursing experience specifically in home care
- Willingness to travel locally for patient visits
- Valid driver’s license, reliable vehicle & insurance
- Live within the service area
- Lift, pull, and push 50-100lbs
Responsibilities
- Manage a weekend caseload within your local territory
- Conduct skilled nursing visits, assessments, and follow-ups
- Develop and adjust personalized care plans
- Partner with physicians and care teams to ensure continuity of care
- Educate patients and families to promote independence and safety at home
