About The Position

Jefferson Healthcare is seeking a full-time Home Health and Hospice Aide (CNA) to join their accredited Home Health & Hospice program. This role involves providing direct personal care to patients in their homes, following a plan of care established by the RN Case Manager. The CNA is essential in delivering compassionate care, promoting patient comfort, and upholding patient dignity.

Requirements

• High School diploma or GED equivalent required
• Current Washington State Nurse Assistant Certified (NA-C) required
• Current AHA BLS/CPR (Healthcare Provider version) required
• Current valid state driver's license and proof of auto insurance required

Responsibilities

• Providing direct personal care to patients in their homes.
• Following a specific plan of care prepared by the RN Case Manager.
• Ensuring patients receive the best possible care.
