About The Position

You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Expect to be hands-on -- providing personal care, supporting daily living needs, and making a real difference in someone's day, every shift. This is a role for someone dependable, patient, and service-driven who wants meaningful work and flexible opportunities. Requirements

  • High school diploma or equivalent.
  • Completion of a state-approved Home Health Aide (HHA) training program, CNA certificate, STNA license or at least 1.5 years of recent caregiving experience in home care, assisted living, long-term care, nursing home, or hospital settings.
  • HHA certification or equivalent credential as required by Ohio state regulations.
  • Basic knowledge of patient care and safety protocols.
  • Ability to pass required background checks and health screenings.

Nice To Haves

  • CPR and First Aid certification.
  • Familiarity with Electronic Health Record (EHR) systems.
  • Strong interpersonal and communication skills.
  • Experience caring for seniors or individuals with chronic conditions, disabilities, or dementia.

Responsibilities

  • Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
  • Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
  • Prepare meals and assist with feeding as needed
  • Support medication self-administration through reminders (no independent med administration)
  • Monitor and report patient condition and changes to the supervising nurse
  • Take and record basic vital signs when trained and assigned
  • Maintain a clean, safe, and orderly client environment
  • Assist with linens and light client laundry as appropriate
  • Use safety equipment properly (rails, supports, stools, etc.)
  • Document care provided according to company and regulatory standards
  • Travel reliably to patient homes and assigned visits across your service area
  • Complete required in-service training, including annual dementia education
