About The Position

The Home Health Aide (HHA) / Certified Nursing Assistant (CNA) provides compassionate, client-centered personal care and supportive services to clients in their homes. Working under the supervision of the Nursing Supervisor and the direction of the Scheduling Coordinator, the HHA/CNA helps maintain the client’s comfort, safety, and dignity while promoting independence.

Requirements

Current Home Health Aide certification or Certified Nursing Assistant license in Massachusetts.
CPR certification (current and valid).
Excellent interpersonal skills and ability to work independently.
Reliable transportation and willingness to travel within the service area.

Nice To Haves

High School Diploma or equivalent (preferred).
One (1) year of experience in a home care setting preferred.

Responsibilities

Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, ambulation, and feeding.
Provide assistance with prescribed exercises and mobility (as trained and permitted by regulation).
Provide medication reminders as directed by nursing supervisor.
Take and record vital signs, observe and report changes in client condition to the Nursing Supervisor as directed.
Prepare meals and assist with feeding as needed.
Perform light housekeeping to support a safe and clean home environment.
Provide companionship and emotional support.
Accurately document care provided and report changes in condition or concerns promptly.
Use GPS/EVV app and company software for clocking in and out of shifts.
Communicate regularly with the Scheduling Coordinator regarding assignments, availability, and scheduling needs.
Follow the individualized care plan as directed by nursing staff.
Provide companionship, socialization, and emotional support to clients.
Assist with meal planning and preparation according to dietary guidelines.
Perform light housekeeping duties such as laundry, dishwashing, vacuuming, dusting, and general tidying.
Assist with errands, grocery shopping, and escort to appointments or social activities (as applicable).
Provide reminders for medications(without administering), appointments, and hydration.
Observe and report changes in the client’s condition or home environment to the Nursing Supervisor.
Communicate effectively with the Scheduling Coordinator regarding schedule changes, availability, and client concerns.
Ensure the client’s living environment is safe and free from hazards.
Adhere to agency policies, procedures, and applicable state regulations.
Maintain client confidentiality in accordance with HIPAA guidelines.
Attend required training sessions and in-service education.
Maintain professional boundaries and uphold the agency's code of conduct.
