Requirements
- High school diploma or GED preferred.

Responsibilities
- Assists with implementation of the domestic plan of care which may include but is not limited to; preparing meals, running errands, house cleaning and doing laundry.
- Maintains accurate documentation in accordance with policies and procedures.
- Communicates participant concerns or changes in participant status to supervisor.
- Home Health services may be provided in a variety of locations including but not limited to participant residence.
