Requirements
- Active HHA Certification in the state of New York
- Government Issued ID, Social Security Card & Work Authorization, if needed
- 2 Recommendation Letters
- Recent Physical
- TB Screening (Quantiferon preferred)
- Chest X-Ray if Positive
- Rubella & Rubeola titer lab work (MMR'S)
- Chain of Custody Drug Screen (Not Neccesary)
