About The Position
Traditional Home Health Care is hiring compassionate, dedicated Home Health Aides (HHA) to provide one-on-one care to patients in Luzerne County. Traditional Home Health Care is a leader in the health care field, providing skilled nursing/aide services and state-of-the-art educational programs. Our goals are to restore patient health and increase their confidence in their abilities to lead healthier, more productive lives.
Requirements
- Compassionate and dedicated
- Ability to provide one-on-one care
- Ability to interact as a member of a multidisciplinary team
- Ability to report any changes in patient condition to agency/nurse case manager
Responsibilities
- Performs personal care/ household duties (e.g. dressing, toileting, feeding, light housekeeping, hygiene, application and removal of splints etc.) in home/school setting
- Maintain a clean and safe environment
- Provide emotional/behavioral support
- Follows plan of care
- Ability to interact as a member of a multidisciplinary team
- Reports any changes in the patient condition to agency/nurse case manager
