Requirements

High school diploma or equivalent required
Ability to pass background check and drug screening

Nice To Haves

Previous Home Health Aide or caregiving experience preferred
Home Health Aide (HHA) certification preferred or willingness to obtain certification in accordance with New Hampshire regulations
Reliable transportation preferred
Ability to work independently while collaborating with a healthcare team

Responsibilities

Assist patients with activities of daily living including bathing, dressing, grooming, toileting, and mobility support
Provide companionship and emotional support to promote patient well-being
Monitor patient condition and report changes or concerns to nursing staff or supervisors
Assist with medication reminders and basic household tasks as needed
Maintain accurate records of care provided and communicate effectively with healthcare team members and family caregivers
Follow individualized care plans and instructions carefully
Adhere to all safety, confidentiality, and infection control procedures
