Requirements
- Comfortable and confident visiting patients in their homes across the Park Rapids and surrounding areas.
- Dependable transportation to provide timely and reliable care.
Responsibilities
- Provide essential support that empowers clients to maintain their independence.
- Assist clients with everyday activities, including mobility support and personal hygiene.
- Ensure clients live in a clean and safe environment.
- Forge genuine connections with clients, becoming a trusted part of their lives.
- Collaborate closely with Nurse Case Managers to promptly address and report any health concerns.
